Online Registration Forms for 2009-2010
The
Main Office will be open from 8:30AM to 3:30PM, Mon ~
Friday. Parents or Guardians of new students
are required to bring registration documents for
approval . Please call or
e-mail Mrs. Verrier at
the Main Office at 887-3621 x302 for more info.
Registration Documentation
Please bring the following documentation to
registration:

Child's Birth Certificate (This should have the seal
on it. The Office will make a copy and return the
original to you.)

Proof of Residency (lease agreement, purchase and
sales, recent utility bills, etc) In the case of
living with a related or unrelated individual, a
notarized statement is required stating who the
parent/guardian and student(s) live with. Also a
recent utility bill in that person’s name must
accompany the statement.

Up to Date Immunization Records and a recent Proof
of a Physical Examination within in a year of
entering Chester Academy.

Court Custodial Papers or Other Legal Documents if
applicable.
